RER/9/098: Reg Tech WS Event Analysis & Root Causes, including HOF

From 9-13 November 2009, the project RER/9/098 Improving Safety Management Systems and Operation Feedback, will hold a Regional Technical Workshop on Event Analysis and Root Causes, including Human and Organizational Factors (HOF)

PURPOSE OF THE WORKSHOP: There are several ways to improve nuclear safety; one way is to learn from events through root cause analysis. A precondition for this is that the root cause analysis is performed in a professional manner. The analysis need to be profound and expose the roots of the event to become effective. When digging down to the roots of an event it will demonstrate that 80-90% of the investigations are related to human and organisational factors. The general objective of the workshop is to exchange and disseminate knowledge on root cause analysis.

This workshop will focus on root cause analysis with emphasis on human and organisational factors. Following subjects will be discussed:
• Fundamentals of root cause analysis
• Different root cause analysis methods
• Human and organisational factors
• Proactive and reactive root cause analysis
• Barriers linked to human and organisational factors
• Cultural aspects related to root cause analysis
• Corrective actions
• Learning from events

The subjects will discuss both from a theoretical and practical point of view.

PARTICIPANTS QUALIFICATIONS: The participants should be professional staff members of the regulatory
authorities, utilities and design or technical support organizations from the region.

The participators should have minimum five years of proven experience of event analysis in role of orderer, investigator, reviewer, decision maker, and/or evaluator. Candidates shall provide nomination forms with clear indication of their experience in the field of event cause analyses. The IAEA’s will also consider candidates from countries embarking nuclear power already engaged in preparatory work.

NATURE OF THE WORKSHOP: The Technical Workshop will consist of plenary presentations and discussions in working groups. Participating countries are expected to present their experience/approach for event analysis. IAEA will send a
standard template for preparation of the national presentations. The presentations shall be in forehand sent to IAEA and touch one of following subjects based on experience:
o Positive and negative aspect of the RCA method
o Lessons learned from RCA (good and negative lessons)
o Experience from proactive RCA
o Successful corrective actions
